某年某月某日晚上，正线有一个线路巡检施工，作业区域：乙站（不含乙站下行）～甲站下行线。乙站通知需在甲站下行头端设置红闪灯防护。甲站行值告知值站设置信息，但值站错误听成是上行头端设防护，就下站台把红闪灯放在了上行头端轨道内。因没有携带对讲机值站设好红闪灯后没有立即报行值。甲站行值看到值站从站厅走回来，也未询问防护设置位置是否正确，就直接在IBP盘上的线路施工示意图的下行头端设置了一个防护标志，值站也未发现IBP盘上防护设置的位置与实际位置不符。随后调试车到达甲站时，司机在进站前发现了上行头端轨道内的红闪灯，便在站外停车，并呼叫车控室。此时行值才发现红闪灯放错位置，通知值站马上撤除，并重新将红闪灯放到下行头端的正确位置。该事件发生后，车站当班人员没有按程序及时将信息上报。
